Letter of Intent to Apply for Grizzlies Foundation Charitable Grant
Deadline: June 4, 2012
Please submit letter on organizational letterhead which contains the following in 700 words or less:
Focus Area A: Organizations that currently operate a youth mentoring program:
- Organization name
- Executive Director/ CEO name and contact information
- Mentoring Program name
- Mentoring program contact name/ information (if different from ED/ CEO)
- Annual organization budget
- If different from above, Mentoring Program Budget
- Purpose, mission and goals of organization
- Description of youth mentoring program, including but not limited to: youth targeted, number of matches maintained, staffing/ support for program, volunteer recruitment strategy, youth outcomes achieved, funding and community partnerships that support program, etc.
- Number of proposed new mentor/ mentee matches for 2012-2013
- Description of how Grizzlies Foundation funds will be used.
Focus Area B: Organizations that contribute significantly to the recruitment and/ or retention of youth mentors.
- Organization name
- Executive Director/ CEO name and contact information
- Person responsible for mentoring project/ recruitment and contact information (if different from ED/ CEO)
- Annual organization budget
- Purpose, mission and goals of organization
- Explanation of how youth mentoring fits within mission/ vision of organization
- Approach/ strategy taken to recruit and retain volunteer mentors
- Future organizational ambitions/ goals related to youth mentoring
- Description of how Grizzlies Foundation funds will be used.
Focus Area C: Organizations that are strategically aligned with mentoring programs by providing specialized and significant expertise and support in program development.
- Organization name
- Executive Director/ CEO name and contact information
- Mentoring project contact name/ information (if different from ED/ CEO)
- Annual organization budget
- Purpose, mission and goals of organization
- Explanation of how youth mentoring fits within mission/ vision of organization
- Explanation of organization’s interest in youth mentoring and how mentoring work is related to core mission of organization.
- How do youth, volunteers, and/or organizations benefit from your support of youth mentoring? Explain how this work has benefited each of these.
- Description of how Grizzlies Foundation funds will be used.
